Skills
- Current PA Registered Nurse license; Bachelor’s degree in nursing, preferred
- Previous RNAC and/or ADON exp preferred or previous exp in related position.
- CPR Certified or ability to obtain.
- Excellent computer skills, including EMR & MDS.
- Knowledge of LTC regs & operations, including Federal, State, & Local regulations.
- Demonstrated leadership and supervisory skills.
- Effective & professionally communicate verbally/writing with all stakeholders
- Ability to effectively educate & train co-workers on clinical and non-clinical topics
- Knowledge of infection control regulations and procedures.
- Knowledge of wound prevention, assessment, and treatment.
Responsibilities
- Assist and support fellow nurses in coordinating all nursing services in accordance with SNF Rules & Regulations, Resident Bill of Rights, Nursing Services P&P, and the HV Employee Handbook
- Provide supervision to staff through effective communication, education, coaching and corrective action
- Participate in hiring process
- Actively participate in a variety of internal committees. Attend and participate in QA and PI Activities.
- Participate in meetings: Care plans, Medicare, Clinical Rounds, and others as directed.
- Coordinate and develop in-services
- Assist in reviewing incident reports. Initiate and follow-through on investigations
- Maintain 672 and 802 forms for assigned households. Review census and roster sheets verifying accuracy.
- Submit state reportable incidents as needed.
- Complete weekly wound rounds on pressure areas and as needed, per HV policy
- Serve as Infection Control Officer for the community
- Administer the following: PPD clinic, influenza, Hepatitis B vaccines and titers. Maintain proper documentation, tracking, & compliance with regulations and policy
- Assist with staffing relief and nursing administration support as needed.
- Participate in the On-Call rotation
- Responsible for all aspects of MDS completion in accordance with established regulations and time lines
- Coordinate MDS schedule, coordinate Quality of Life Rounds, & participate as positive & active member of the interdisciplinary team.
- Participate in the admissions process of new residents.
- Review pharmacy report with medical records secretary and resolve as needed.
- Update care plans quarterly and as needed, follow-up on care plan, monitor restorative nursing programs
