Responsibilities/Qualifications
SCN is hiring a Discharge Planner in the West Bloomfield area. As the Discharge Planner you will assist and perform social work and discharge planning responsibilities for residents in a nursing facility. The position prepares the resident to transition from the nursing facility to the next appropriate care setting based on their individual needs. The Discharge Planner applies their understanding of medical conditions, coverage, and initiates an individualized discharge plan.
Experience working in a nursing home, discharging the residents with appropriate services, such as DME equipment, medications, therapy, etc. is a requirement.
Responsibilities:
- Provides medically related social services to assigned caseload that assists the residents to attain or maintain the highest practicable physical, mental and psychosocial well being
- Services provided meet professional standards of social work practice, consistent with state and federal laws and regulations
- Guides facility staff in matter of resident advocacy, protection and promotions of residents’ rights
- Attend interdisciplinary team meetings, care conferences and family meetings
- Complete psycho-social resident assessments, cognitive screenings, and educate resident’s and families about advanced directives
- Knowledge of 3877/78s
- Issue Notices of Medicare Non-Coverage to resident’s and families
- Provide resident’s and families with discharge planning services including ordering durable medical equipment, home health care services, and helping to establish safe discharge plans
Requirements:
- Will accept all levels of Social Work education/license
- Solid organizational and case management skills
- Ability to set priorities, take initiative and meet deadlines
- Good communication skills, diplomacy and tact
- Good decision-making, conflict-resolution and effective listening skills
- Ability to handle crisis, cope with job stress and maintain confidentiality
- Invests a minimum of 75% of work time on referral management, interacting with hospital discharge planning staff daily, providing updates on pending referrals and gathering new referrals
- A background check will be required
