JOB SUMMARY: This position requires strong organizational skills and outstanding people skills that will play a vital role in the success of our integrated health facility. As the Case Manager, you will be responsible for providing individualized support to clients with mental health needs and ensuring the successful coordination of their care with appropriate resources. This position reports directly to the Behavioral Health Services Manager. This position requires a passion for mental health advocacy.
PRIMARY DUTIES AND RESPONSIBILITIES:
- Case Management: Conduct assessments of clients' mental health needs for medical providers as requested. Coordinate and connect clients with appropriate mental health services, resources, and community support systems. Provide ongoing monitoring and follow-up to ensure clients' needs are met and progress is achieved.
- Communication: Must be able to effectively communicate with staff and clients.
- Client Advocacy and Support: Advocate for clients' rights and needs within the mental health system and other relevant services. Provide emotional support and crisis intervention when necessary.
- Data Management and Reporting: Maintain accurate and up-to-date client records to ensure that any gaps in care are addressed.
- Risk Management: Identify potential risks and challenges within case management with support from the Compliance and Risk Coordinator, implement strategies to mitigate them. Collaborate with the Behavioral Health Services Manager on contingency plans for addressing unforeseen issues.
- Compliance and Quality Assurance: Ensure compliance with all relevant regulations, policies, and ethical standards in case management. Maintain high-quality services and adherence to best practices in mental health care. Participate in organizational QA/QI and Risk Management teams.
INTERMITTENT DUTIES:
- Performs other duties as assigned by supervisor.
SKILLS/QUALIFICATIONS:
- Bachelor's degree in Psychology, Social Work, or related field.
- Proven experience in case management, preferably in mental health or related fields.
- Strong organizational and time management skills with the ability to manage multiple priorities effectively.
- Excellent communication, interpersonal, and problem-solving skills.
- Knowledge of mental health programs, services, and community resources.
- Ability to work collaboratively with diverse teams and stakeholders
- Bilingual in English and Spanish is a plus
- Competency in computerized practice management systems and other computer software (MS Word, Excel, Outlook, Internet).
