Responsibilities: Assist Admissions Team by responding to community referrals with information meetings, admissions assessment, and planning. Assist with continuity of care and discharge planning from hospitals. Responsible for identifying patient/family needs, for providing skilled nursing assessment, planning and supportive care, and appropriate services in accordance with the attending physician’s orders and the interdisciplinary group plan of care. Responsible for completion of assigned regular visits, admit or liaison nursing functions based on staffing and patient/family needs, and for assessment and care of patients in end of life care.

Essential Functions:

  • Plan for admission of patients to Hope Hospice in coordination with community liaison nurses, discharge planners, Nurse Supervisors, and Manager of Admissions.
  • Screen and process inquiries for admission to hospice program; make telephone contact and/or assessment visits with potential patients to provide information on Hope Hospice services and/or assess patient’s status; determine patient’s appropriateness for hospice services consistent with Hope Hospice policies and admission criteria and in conjunction with patient’s physician
  • Perform all pre-admission duties associated with admitting a patient to the Hope Hospice program; assume initial responsibility for assessing patient/family needs and consulting with attending physician and other hospice staff members; manage patient/family care plan and accompanying services between admission and transition to case managing nurse
  • Assist with coordination of services required for discharge of patients from hospitals into Hope Hospice program of care
  • Assume primary responsibility for a limited patient/family caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process.
  • Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
  • Maintain regular communication with the Manager of Admissions to review caseload; maintain regular communication with the attending physician concerning patient/family care needs & response to interventions.
  • Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care; seek input from other group members regarding the patient plan of care in order to obtain additional knowledge and support.
  • Maintain up-to-date patient records so that problems, plans, actions & goals are accurately and clearly stated, and changes are reflected as they occur; learn & utilize software program for electronic medical records.
  • Instruct primary caregivers, volunteers, and employed caregivers to provide care as indicated.
  • Attend Interdisciplinary Team meetings and facilitate discussion of issues from caseload for full staff discussion, consultation and evaluation.
  • Supervise and document observance of the home health aide delivery of care as required.
  • Inform the Supervisor of unusual or potentially problematic patient/family issues; meet regularly with hospice nursing staff to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth.
  • Share in on-call rotation, providing 24-hour, seven-day-a-week coverage to patients/families as necessary.
  • Provide appropriate support at time of death and perform bereavement assessment if requested.
  • Assume responsibility for visits as scheduled and consistent with Hope Hospice policies and procedures.
  • Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
  • Maintain communication with the Directors of Patient Care/Clinical Supervisor, attending physician, and case managers concerning patient/family care needs and response to interventions.
  • Inform the Directors of Patient Care/Clinical Supervisor of unusual or potentially problematic patient/family issues.

Basic Qualifications including:

Required Experience

  • Minimum three years of varied work experience as a professional nurse with preferably one of the three years in either an acute, oncology, Home Health and/or Hospice setting.

Education / Licenses

  • Possess a current license to practice as a registered professional nurse in the State of California.
  • Preferably a B.S. degree in nursing with a P.H.N. certificate.
  • Possesses and maintains current CPR certification.

Job Type: Full-time

Pay: $55.00 - $60.00 per hour
