Summary

The Community Navigator works with newly designated members with a Serious Mental Illness (SMI), assisting with transitional care back to the community. This includes members who are being released from incarceration who are involved with the courts, probation or parole, providing transitional care back to the community. Guidance and direction is provided by the Regional Program Manager and Senior Director of Community Programs.

Essential Duties

  • Engages with the courts, probation and parole to identify SMI, NTXIX members in need of transitional care services.
  • Relate own recovery process in a positive and supportive manner through coaching, role modeling and mentoring in order to assist members in developing skills to achieve their goals.
  • Coordinates care with community agencies and assists member in accessing community based services.
  • Provides support to include transportation, education, navigation and assistance with the acquisition of essential needs such as emergency clothing, food, and housing assistance when appropriate.
  • Work with the member to use natural supports and reintegrate into the general community to the maximum extent possible, reducing recidivism.
  • Utilize tools such as the DLA 20 to identify social determinants of health and work with member to address their needs, for a holistic approach to wellness.
  • Complete trackers and other deliverables associated with tracking member engagement, outreach, and outcomes.
  • Assist with AHCCCS enrollments for members who are eligible for Medicaid.
  • Promote HOPE, Inc. as the premier peer support organization to the behavioral health community and prospective funding sources.
  • Conduct community outreach and attend events that promote the program in the community and increase visibility of mental health block grant services.
  • Perform other related duties in accordance with agency growth, departments, and changes.

Additional Duties

  • Meets with new members providing peer support and orientation to HOPE, Inc. site(s).
  • Schedules follow-up appointments for members as appropriate.
  • Answers members’ questions using HOPE, Inc. policies and procedures.
  • Provides transportation for members using a HOPE vehicle as needed.

Qualifications

  • At least 21 years of age
  • High School/GED 2 years’ behavioral health experience or
  • Bachelor’s degree or Master’s degree in a behavioral health field
  • (Required) Peer - Individuals must have first- hand knowledge of a behavioral health condition or substance abuse issue, whether or not they received treatment or medication OR identify as a family member of someone in recovery.
  • If the staff identifies as a peer using the definition listed above, they must attend Peer Support Certification training within the first 3 months of employment.
  • Certification as a Community Health Worker required within 3 months of employment.
  • Demonstrate strong leadership and organizational skills
  • Ability to implement organization-wide activities (setting objectives, developing strategies, developing policies and procedures for programming, and organizing the functions necessary to accomplish the activities)
  • Valid Arizona Driver’s license, Driver’s license is an essential requirement for performing duties for this position with a Clean Motor Vehicle Record - no more than 2 moving violations or a license suspension in past 3 years.
  • Current 1st Aid and CPR certification
  • Proof of TB testing within the last year
  • Fingerprint Clearance, if required by program
