Summary
The Justice Transition Center Liaison promotes a recovery-based approach to member care that emphasizes hope, respect, self-direction and empowerment by providing peer support, education on community resources and other supportive services, to aid in members’ recovery from mental illness and/or addiction disorders. Guidance and direction are provided by the Criminal Justice Program Manager and Senior Director of Community Programs.
Essential Duties
Transition Center Responsibilities:
This position will be co-located at the prebooking modular at the Pima County Adult Detention Complex (PCADC), referred to as the “Transition Center”.
Staff will work in collaboration with pretrial services and Pima County staff, and engage individuals who receive a field release back to the community, with a scheduled court appearance.
Provide peer support, transportation, and navigation services to participants in the program to ensure successful transition back into the community with appropriate resources and support. Relate own recovery process in a positive and supportive manner through coaching, role modeling and mentoring in order to assist members in developing skills to achieve their goals.
Coordinate care with community stakeholders to assist the member in attending appointments, overcoming obstacles related to service accessibility and utilization.
Provide members with education on harm reduction, overdose prevention, and substance-use treatment options for the completion of goals in the program scope of work.
Obtain all necessary Releases of Information (ROI) to include member/potential member support system, and affiliated partners.
Promotes HOPE as the premier Peer Support agency in the community, and refers for outpatient services as appropriate.
Jail Population Review (JPR) Peer Support (“Navigator”) Responsibilities:
Attend weekly the JPR Committee meetings prepared to share program updates and progress.
Initiated peer support services for any individuals referred to JPR Peer Mentor;
Assist in coordinated reentry planning for individuals released via JPR;
Assist Pretrial Services, Probation, and/or other justice system or treatment partners to coordinate services (such as treatment) and resources (such as housing and transportation);
Provide on-going peer support to and maintain regular communication with individuals under pretrial supervision, until sentencing or other logical conclusion (such as transition of care to other program that includes peer support, like Housing First, or participant does not wish to continue participation);
JPR Navigator is encouraged to assist participants by sending reminders of court appearances or treatment appointments, as well as create a plan with client on transportation;
Provide transportation using a HOPE fleet vehicle, when approved by DJS and supervisor;
Performs other duties as assigned in connection with program and needs of HOPE, Inc.
Requirements
At least 21 years of age
High School/GED 2 years’ behavioral health experience or bachelor’s degree or master’s degree in a behavioral health field with no experience.
(Required) Peer - Individuals must have first- hand knowledge of a behavioral health condition or substance abuse issue, whether or not they received treatment or medication OR identify as a family member of someone in recovery.
Individuals must have personal lived experience with the criminal justice system. Individuals must have completed all court requirements and be eligible to receive clearance from the PCADC.
Peer Support Certification training within the first 6 months of employment.
Demonstrate strong leadership and organizational skills.
Ability to implement organization-wide activities (setting objectives, developing strategies, developing policies and procedures for programming, and organizing the functions necessary to accomplish the activities)
Valid Arizona Driver’s license, Driver’s license is an essential requirement for performing duties for this position with a Clean Motor Vehicle Record - no more than 2 moving violations or a license suspension in past 3 years.
Current 1st Aid and CPR certification
Proof of TB testing within the last year
