Description

Promotes a recovery based approach to member care that emphasizes hope, respect, self-direction and empowerment by providing peer support, education on community resources and other supportive services, to aid in members’ recovery from mental illness and/or addiction disorders. Guidance and direction is provided by the Program Coordinator, Clinical or Site Director and the Chief Clinical Officer.

Essential Duties

  • Oversee all aspects of care to assigned members to ensure needs are identified and addressed to best support positive outcomes.
  • Relate own recovery process in a positive and supportive manner through coaching, role modeling and mentoring in order to assist members in developing skills to achieve their goals.
  • Work with recovery team members to assess mental and physical health and psychosocial needs and work collaboratively with the member/family, health home, community resources and other natural supports to develop an Individualized Service Plan (ISP) to address needs.
  • Ensure all identified services are offered and delivered to meet identified needs and objectives and expected outcomes are achieved.
  • Coordinate identified needed services internally within HOPE such as therapy, employment, and Substance Use Disorder services.
  • Facilitate communication and coordination between participants of the health care team, involving the members in the decision-making process in order to minimize fragmentation in the services.
  • Work with the member to use natural supports and reintegrate into the general community to the maximum extent possible.
  • Respond to members and their family when a member is hospitalized, arrested, or when other major life events occur to ensure services are appropriate and to assist the member in the safe transition of care to the next most appropriate level. Communicate with the member’s health home including care around major medical conditions to ensure effective coordination of care.
  • Track when assessments, ISPs, and crisis prevention plans need to be updated and ensure they are completed within the appropriate timeframes.
  • Provide outreach to members who are not engaging in services as expected and communicate lack of participation to other recovery team members as appropriate.
  • Provide support, rehabilitation and counseling services to any member of HOPE, Inc. in an individual or group setting, at clinical sites or in the community as indicated on the member’s ISP.
  • Complete timely documentation within members’ behavioral health records in accordance with agency and funding source requirements.
  • Ensure compliance with and support of Executive directives, Agency contracts and established policies and procedures.
  • Promote HOPE, Inc. as the premier peer support organization to the behavioral health community and prospective funding sources.
  • Perform other related duties in accordance with agency growth, departments, and changes.

Additional Duties

  • Meets with new members providing peer support and orientation to HOPE, Inc. site(s).
  • Schedules follow-up appointments for members as appropriate.
  • Answers members’ questions using HOPE, Inc. policies and procedures.
  • Provides transportation for members using a HOPE vehicle as needed.

Requirements

  • At least 21 years of age
  • High School/GED 2 years’ behavioral health experience or

Bachelor’s degree or Master’s degree in a behavioral health field

  • (Required) Peer - Individuals must have first- hand knowledge of a behavioral health condition or substance abuse issue, whether or not they received treatment or medication or identify as a family member of someone in recovery.
  • If the staff identifies as a peer using the definition listed above, they must attend Peer Support Certification training within the first 3 months of employment.
  • Demonstrate strong leadership and organizational skills
  • Ability to implement organization-wide activities (setting objectives, developing strategies, developing policies and procedures for programming, and organizing the functions necessary to accomplish the activities)
  • Valid Arizona Driver’s license, Driver’s license is an essential requirement for performing duties for this position with a Clean Motor Vehicle Record for the last 39 months when applying.
  • Current 1st Aid and CPR certification if required by program.
  • Proof of TB within the last year

Typically, these skills and knowledge are the result of a combination of practical education in peer support and several years of experience in personal recovery, either directly or as a family member.
