JOB: Jail Re-Entry Coordinator
Reports to: Program Director
ESSENTIAL FUNCTIONS:
· Provide substance use and recovery support services inside of the Portage County Jail.
· Contribute to development and implement a reentry plan collaboratively with persons served. Plans include identifying reentry client’s basic needs (i.e. housing, insurance, treatment, transportation, recovery supports, etc..) problems, goals, and measurable objectives.
· Complete and manage assigned documentation, reports and relationships with referents.
· Effectively communicate with community partners (i.e Courts, Probation, treatment providers, JFS, recovery housing, etc…)
· Follow all outlined processes and procedure systems outlined when communicating with entities inside and outside of the jail on behalf of the clients residing in the jail.
· Follow all rules and expectations outlined by jail administration.
· Perform all services with a trauma informed client centered approach to care.
· The ability to know and understand that there are multiple pathways to recovery and willingness to explore this from a client center approach.
· Continually monitor and evaluate each client’s progression through their reentry plan, and develop corrective action revisions to the plan as needed.
· Coordinate care with all other professionals involved in client's case from other outside agencies.
· Knowledge and understanding of the treatment of mental health and addiction as co-occurring disorders and ability to implement in care for client.
· Contact supervisor or designated back-up for clarifications of procedures or clinical assistance as needed.
· Maintain a network of professional relationships with relevant service providers in Portage County.
· Work closely in collaboration with clients to engage them in the decision-making process for goal setting, post-incarceration.
· Complete paperwork requirements and maintain quality, up-to-date records,
· Participates in regular supervision, jail meetings, team meetings, staff in services and ongoing training as needed and also maintains all required hours for license/ certification renewal.
· Always maintains professionalism.
· Other duties, as assigned.
QUALIFICATIONS:
· Associate degree or higher.
· State of Ohio License or Certification for work with substance use disorder. Acceptable licenses/certifications are: Certified Ohio Peer Recovery Supporter, CDCA, LCPC II or LCPC III. Must maintain a professional license/certification and stay current (not revoked, suspended, or lapsed in registration), valid and unrestricted.
· Ability to work in a fast-paced environment.
· Ability to independently time manage
· Minimum of 1-year experience in a treatment/jail setting, 2 years’ experience preferred.

Pay: $32,000-$35,000
