RN Position Summary:
The Registered Nurse (RN) Coordinator helps ensure that the referral and admission process functions smoothly and efficiently, with a focus on customer service. The primary role of the RN Coordinator is to review records to ensure that all aspects of supporting documentation are present when discussing hospice eligibility with Hosparus Medical Directors (HMDs). The RN Coordinator will serve as a clinical resource on the Referral & Admissions team and will focus on preparing each medically eligible referral for admission prior to the first visit. In addition, the RN Coordinator assists in identifying patients who may benefit from a referral to one of Hosparus Health’s palliative care programs.

RN Position Duties & Responsibilities:
- Reviews records obtained by the Referral Coordinator(s) (RC) and determines if all information and supporting documentation is present to obtain HMD verbal certification for medically eligible hospice patients.
- Obtains physician order(s) or verbal certifications for referral and admission, as needed.
- Assists in identifying patients who may benefit from a referral to Hosparus Health palliative care if not medically eligible or not electing the hospice benefit.
- Accurately and timely completes all documentation and forms (preadmission note) for referral calls and admissions processing.
- Reviews referral data to ensure accuracy prior to RC scheduling visit.
- Collaborates with RCs as needed when situations require clinical judgement and review of in-depth hospice services with patients, family and/or referral sources.
- Review of and/or documentation of medication and other pertinent orders to ensure continuity of care.
- Communicates with referring MD office or specialists to obtain information or clarify any questions as needed.
- Works autonomously yet recognize the need to involve upper management and/or other disciplines.
- Builds collaborative and collegial relationships with Referral and Admissions team members, interdisciplinary teams, Preauthorization team members, Finance Department, community and referral sources, the Outreach Team and others to assure coordination and continuity of care and services.
- Works with the Supervisor to assure completion of duties, scheduling and arranging coverage for department.
- Serves as a professional mentor to other team members and utilizes knowledge to empower others and improve team efficiency.
- Assumes the role of preceptor when requested.
- Mediates and solves problems effectively and includes supervisor when barriers to admission are present.
- Collaborates with management to ensure effectiveness of Admissions team.
- Understand and have ability to clearly and concisely explain various payor sources and the requirements for coverage based on their guidelines.
- Assess for needs including non-required services (CNAs, volunteers, etc.) and inform Referral Admissions Coordinators (RACs) of anticipated needs.
- Complete preadmission forms on all incoming referrals to streamline admission at RAC visit.
- Identify potential improvements in departmental processes in an effort to increase access and promote efficiency.
- Demonstrates knowledge of hospice admission criteria and is able to communicate the hospice philosophy in providing information to patients and families, referral sources and the public.
- As appropriate during referral process, identify patient/family needs and further escalate the referral to the admission phase.
- Coordinate with admissions, medical and referral staff when patient needs warrant an immediate admission visit.
- Coordinate and communicate with outreach team when community education needs are identified during referral process.
- Completes other duties as assigned by supervisor.
- RN with current RN licensure in the state of Kentucky and/or Indiana as required by designated position, or ability to obtain required license upon hire.
- One year of nursing experience; ER, ICU, home health experience a plus.
- Ability to function independently with little or no supervision.
- High level of computer skills to navigate multiple screens to gather information needed for admission.
- Knowledge of Medicaid/Medicare reimbursement helpful.
- Knowledge of palliative and/or hospice services helpful.
- Use of technology-based tools, including electronic medical record, strongly preferred.
