Job Posting for Director of Community Based Services at Hospice Buffalo
Responsible for the overall day-to-day management of home and community based services of the agency. Provides clinical and administrative direction for the managers and supervisors of hospice community based services. This responsibility includes ensuring compliance with Hospice criteria and regulations as well as state and federal regulations as they apply to the Hospice & Palliative Care programs. Assures that the patients and families receive quality care in accordance with recognized standards of practice, Hospice and Palliative Care criteria, policies and procedures, and both state and federal regulation.
Essential Duties and Responsibilities:
Handles recruiting, hiring, mentoring, evaluating, and separating managers under the Director's supervision.
Maintains working knowledge of state and federal regulations as they apply to hospice and palliative care services.
Responsible for both new and revised policy and procedure development and demonstrates ability to operationalize to assure high quality and regulatory compliance standards.
Assists in the preparation of the annual budget and promotes cost containment within those areas of supervisory oversight including budget variance analysis.
Assists in the implementation of an agency wide Continuous Quality Improvement Program and assists in multiple monitoring activities that are both patient and organizational focused to promote clinical quality and corporate compliance.
Uses leadership position to set positive, attainable expectations, objectives and goals for staff and manager development.
Provides guidance and support to managers, in the planning of patient care, to ensure that such care is rendered as components of an interdisciplinary team and handles any patient care problems that arise including appropriate risk management interventions.
Acts as the first administrative responder to patient/family complaints, unusual occurrences, and risk management issues generated on teams assigned as direct reports.
Oversees productivity standards and staffing levels appropriate to all disciplines and relevant to the acuity of patient caseloads of departments under own purview.
Provides tracking data and other program metrics as requested.
Participates in the 24 hour 7 day per week clinical administrative supervisory coverage.
Qualifications:
Education/Certification
RN required from an accredited school of nursing; BSN or Master's Degree in Nursing and/or Administration preferred.
Current registration with the State of New York to practice as a Registered Professional Nurse.
Must have a current driver's license, free of serious violations, and vehicle required.
Experience
Minimum of two (2) years of management experience with home care supervision required.
Minimum of three (3) years of clinical nursing experience required. Experience in Hospice & Palliative Care preferred.
Experience working in various healthcare settings and electronic documentation helpful.
