Duties & Responsibilities:
- Show up on time to receive a report from the departing nurse.
- Follow the plan of care according to the RN Case Manager.
- Promote comfort measures including the use of comfort medications per MD orders.
- Document at a minimum of one entry per hour on the patient's condition and interventions used.
- Report time of death to hospice.
- Provide emotional support for a patient's family.
- Contact the mortuary and assist with providing emotional support to the patient's family.
- Get the route sheet signed and submit your route sheet.
Requirements & Qualifications:
- Must be able to work in California.
- Must have an active LVN License.
- Reliable and flexible.
Job Details:
Rate: Per-hour rate for continuous care shifts.
The rate differs depending on the experience.
Many other opportunities also exist with varied compensation.
