Hospice of San Joaquin is the oldest not-for-profit hospice of 40 years. We serve all of San Joaquin and Stanislaus Counties. We are seeking a Dynamic, self-motivated RN Case Manager Float - Palliative/Hospice Care, who thrives in a fast-paced environment and supports our mission.
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily and be compliant with respect to nondiscrimination under the Hospice of San Joaquin Personnel Policy and Procedures Handbook. Additionally, the individual must be able to meet with individuals regardless of race, color, religion, sex, national origin, disability or any other characteristic protected by law or that of his/her relatives, friends or associates.
EDUCATION and EXPERIENCE
Graduate of an accredited school of nursing in the State of California as a Registered Nurse. Three years of recent clinical experience. Computer skills. Ability to learn and assimilate the clinical computer program system. Knowledge of advancing chronic or life-limiting illness/conditions, signs/symptoms, treatment interventions, and medication for palliative care. Ability to handle multiple projects and tasks simultaneously; uses time and resources efficiently to accomplish work objectives. Ability to work in a high-stress environment or with participants who are under stress and maintain a positive work attitude while working with customers/participants, staff, and the community.
LANGUAGE SKILLS
Excellent verbal and written communication skills. Ability to analyze, and interpret general business periodicals, professional journals, technical procedures, or government regulations. Ability to write reports. Ability to effectively present information and respond to questions from groups or managers, clients, customers, and the general public. Demonstrated ability to write in a clear, understandable, and concise manner.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written or oral form. Ability to deal with problems involving several concrete variables in standardized situations.
CERTIFICATES, LICENSES, REGISTRATIONS
Possess reliable transportation and have a valid California driver’s license and have automobile insurance which meets State standards. All employees who drive need to provide Hospice of San Joaquin with evidence of insurance. Possess a license to practice as a registered nurse in the State of California.
