Job Description
The social worker provides skilled social services to patients and their families on an intermittent basis in the patient’s home or facility. These services are performed in accordance with the physician orders, plan of care, and are coordinated with member of the interdisciplinary team. The Social Worker is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions of the employment agreement and providing care based on the Social Work Practice Act. The BSW is supervised by the MSW.
Job Duties
Provide initial psychosocial evaluations, ongoing psychosocial counseling, and direct casework services.
Carries out social evaluations and plans intervention based on evaluation findings.
Maintains clinical records on all patients referred to social work.
Serves as a liaison between patients or families/caregivers and community agencies.
Maintains collaborative relationships with organization personnel to support patient care.
Provides information and referral services for patient and family/caregivers regarding practical and environmental needs.
Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risk for suicide, neglect or abuse and plans intervention based on evaluation findings.
Counsels’ patient and family/caregiver as needed in relationship to stress and the other identified coping difficulties. Provides crisis intervention when necessary.
Assess for, and educates interdisciplinary team, on nay special needs related to the culture of the patient and family. Includes communication, role of family, space, and any special traditions or taboos.
Educates patients and families on, and assists in, preparation of advanced directives.
Qualifications/ Requirements
Must have an active state license as a social worker (BSW) in good standing in the state of Arizona.
Must have a current driver’s license, reliable transportation with the ability to travel to make patient visits.
Ensures protection of confidential information that is stored, handled, or maintained by the Agency per HIPAA regulations.
The ability to read and following written and verbal instructions, document care, and have great communication skills.
Basic computer skills and some knowledge in electronic medical records (EMR) systems.
Previous hospice experience as a social worker of two year or longer preferred.
The understanding of the hospice philosophy and being comfortable providing specialized care to the terminally ill.
