What are the responsibilities and job description for the PRN RN Case Manager position at Hospice?
Coverage includes Manatee and Northern Sarasota Counties.The PRN RN Case Manager creates and maintains a plan of care, serving as the coordinator for patient and family interaction with Tidewell for the duration of the patient’s hospice experience. When a patient and loved ones think of Tidewell Hospice, they will think first of the RN Case Manager. The case manager must be able to listen, observe, communicate and sensitively provide nursing services in emotionally intense situations.
The PRN RN Case Manager reviews and updates the care plan, physician orders, pharmacy profile and other records. The nurse also educates patients, family members and other caregivers about the disease process and decline, available palliative interventions, the dying process and safety practices.
The successful applicant will serve hospice patients and their families in the home, long-term care facility, or assisted-living facility.
Applicants must be a graduate of an approved nursing program and own a current Florida RN license. BSN or MSN preferred. Minimum one year experience in an acute-care hospital environment providing direct patient care; previous experience as a hospice RN preferred.
