Overview:
Housing Works Health Home Care Coordination program works in partnership with medical and behavioral health providers to align services that promote access to care and enhance health outcomes for Medicaid recipients with a history or risk of over-utilization of medical and behavioral health services. The Case Management Supervisor guides the efforts of Care Managers and Care Navigators to engage, assess, develop and implement a plan of care that prioritizes the clients’ goals and service needs. The Unit Supervisors employ a case management supervision model using a variety of methods, including individual and group clinical supervision, coaching, training, and data driven supervision to assist Care Managers and Care Navigators improve their skills and effectively meet client’s needs. As key members of the program management team, Unit Supervisors should demonstrate passion and enthusiasm for supervision, and embrace concepts of teamwork, collaboration, and quality improvement. Care Management Supervisors are provided weekly supervision, internal and external training opportunities, and the ability to work as part of an interdisciplinary team of health care professionals.
Responsibilities:
Provide clinical and administrative supervision to care managers to assist with the development of effective interventions and treatment plans that meet client’s needs.
Routinely update staff on updated program and agency policies and procedures.
Participate in ongoing internal and external trainings aimed at enhancing program standards, practice, and client outcomes.
Monitor delivery of services for compliance with programs standards
Provide crisis intervention to clients in psychological distress.
Actively participate in staff meetings to discuss programmatic or agency priorities.
Engage staff in meetings and small groups to elicit input, feedback, and suggestions.
Review and approve client documentation for care management team and provide feedback to team as needed and ensure client charts reflect goals and need of client through the assessment, care planning, and on going care management process
Utilize departmental reports to ensure billing targets and process metrics are achieved.

Occasional field work may be required as needed to achieve client outcomes and health home requirements
Engage in advocacy to end the dual crises of homelessness and AIDS.
Performs other duties as required
Master’s Degree in Social Work, (LMSW), Nursing (RN), Psychology, MPH, Human Services or other related field required
Minimum two years supervisory and administrative experience
Ability to supervise, manage and provide leadership to all staff members and trainees
Adept with word processing, Excel, and Microsoft Office products; Previous experience with Electronic Health Records a plus
Ability to communicate well both written and oral
Good organizational and multi-tasking skills
Strong analytical skills centered around collecting, organizing, and disseminating information
Positive attitude and adaptable with both staff and clients
Highly organized with the ability to work independently and prioritize multiple responsibilities in a fast-paced environment
Comfortable working with a diverse group of staff and clients
The following additional competencies are viewed as important to success in this position:
Proficiency and comfort with online meeting software such as Zoom and Microsoft Teams
Understanding of chronic medical conditions, homelessness, substance use and mental illness
Knowledge of City, State, and Federal entitlement and benefit systems.
Essential Physical Demands/ Working Conditions:
Ability to travel within the boroughs via public transportation as needed and ability to work from remote office locations.
Requires access to email, phone, Zoom and Teams.
Requires ability to comply with agency confidentiality guidelines in remote workspace
