AGENCY DESCRIPTION: 

Huckleberry Youth Programs (HYP) is a critical provider of essential safety net services for high-needs youth and their families in San Francisco and Marin County. For over 50 years, HYP has developed and maintained prevention programs that promote the health and leadership of youth, and, as a result divert youth from violence, crime and gang involvement, as well as other harmful behaviors, including running away, drug/alcohol abuse, risky sexual activities, and involvement in the juvenile justice systems. POSITION SUMMARY:

The primary responsibilities of this position is to ensure that youth working with CARC receive caring and professional case management services; assist youth and their families with the implementation of recommended treatment plans so they are able to re-focus their attention into positive and productive behaviors and choices. The Case Manager must be able to work within a collaborative process and negotiate successfully with multidisciplinary systems. This position works mostly in person between 11:30 a.m. and 7:30 p.m. M-F.  The Case Manager will be responsible for, but not limited to the following:

ESSENTIAL FUNCTIONS:

  1. Conduct intake assessments with youth, including interviewing parents, guardians, and/or other involved adults.  

  2. Provide short-term interventions and ongoing case management services for youth.  

  3. Engage and support the participant's family regularly.  

  4. Support youth in meeting their treatment goals.

  5. Create and maintain client files

  6. Network with other youth providers to establish liaisons and resources.

  7. Maintain openness to different experiences and work with youth/families of differing ages and backgrounds

  8. Maintain ongoing caseload and consistent documentation of case management contacts and client progress.

  9. Make official contacts with schools, counselors, probation officers, attorneys, and others associated with the youth.

  10. Communicate with probation officers on youth’s status. Attend probation meetings & court dates.

  11. Participate in weekly client reviews, monthly staff meetings and other agency related meetings as directed by the Program Director.  

  12. Participate in all trainings, community conferences and meetings involving Juvenile Restorative Community Conferencing program

  13. Meet youth in the field with flexible scheduling.

  14. Perform tasks and responsibilities in a complete and timely manner, complying with agency policies and standards and conforming to scheduling requirements of this job and program.

  15. Maintain a creative, team-building, cooperative approach to job performance and seek to bring a constructive problem-solving orientation to all tasks.

  16. Phone coverage including answering and responding to incoming crisis, business, informational and referral calls.  

  17. Other duties as assigned.

COMPETENCIES:

  1. Experience successfully working with disenfranchised youth

  2. Excellent organizational and writing skills required

  3. Excellent collaborator who is adaptable and is committed to open communication  

  4. Knowledge of system collaboration preferred

  5. Knowledge of community based agencies serving adolescents in S.F strongly preferred

 

SUPERVISORY RESPONSIBILITIES:

This position has no supervisory responsibilities.
 

PHYSICAL DEMANDS:

This is largely a sedentary role; however, some filing is required. This would require the ability to lift files, open filing cabinets and bend or stand as necessary and climb stairs.


 

 

Qualifications


REQUIRED EDUCATION AND EXPERIENCE:

  1. A combination of three years of college credits and/or relevant experience working with the SFUSD or SF Juvenile Justice system preferred

PREFERRED EXPERIENCE:

  1. Ability to perform core responsibilities in English/Spanish. Stipend provided.

ADDITIONAL ELIGIBILITY QUALIFICATIONS:

  1. TB clearance

  2. Valid CA driver’s license & clear DMV record 

  3. All employees must be fully vaccinated for COVID-19 unless approved for exemption for medical or religious beliefs

SECURITY CLEARANCE:

  1. Must pass DOJ/FBI background check. Due to our requirements with licensing, candidates with backgrounds including items on this non-exemptible crimes list will not be eligible for exemption at this time.
