DESCRIPTION OF POSITION:
Meet with psychiatric clients to take and record vitals in medical record, review and record accurate up-to-date medication list and update patient and family history with the client, complete overflow documentation as requested by psychiatric providers and supervising nurse, and organize all medical records for psychiatric provider review. Will provide data entry for any incoming lab work or medical records from outside physicians in client medical record after review by the supervising nurse. This position will also work with individuals in the Behavioral Health and New Horizons area to provide coordination of care in medical services being provided to eligible individuals.

Position Details:
Full Time: 32-40 hours per week (Monday - Friday)

Job Knowledge and Comprehension:
A. Basic knowledge of taking vitals.
B. Computer knowledge of HSA EHR.
C. Receive phone calls from clients who have called with questions regarding their prescription or side effects from medication. Route questions to medical director and/or mid-level practitioner if delegated by supervising nurse.
D. Coordinate scheduling or appropriate appointments or consults with medical director and/or mid-level practitioner regarding treatment options and relays this information to client's pharmacy if needed.
E. Reports lab findings to medical director, mid-level practitioner and/or supervising nurse.
F. Organize medical records from outside entities for psychiatric providers before evaluations on new clients as well as relay incoming medical information on existing clients.

G. Develop a person centered care plan for each individual that coordinates and integrates all of his or her clinical and non-clinical health care related needs and services.

H. Will complete documentation in the FFT system as well as the HSA clinical record.

I. Complete bi-annual reports for New Horizons.
J. Other duties as assigned consistent with non-exempt functions.

Physical Requirements:
Required to sit for long periods of time. Infrequent light physical effort required lifting up to 10 pounds occasionally and 25 pounds rarely. The employee is frequently required to sit and reach with hands and arms. Employee may be required to work extended periods of time at a computer monitor. Must be able to drive vehicle occasionally.

Required - High School diploma or GED.
Preferred - South Dakota licensed practical nursing, medical assistant certification, or EMT/paramedic certification.

The Board of Directors of the Human Service Agency shall have the prerogative to hire staff whose qualifications may vary from the above. This position requires access to Electronic Personal Health Information (EPHI). Current HSA Policies and Procedures apply. It is the responsibility of the employee to read, understand, and comply with HSA policies and procedures that apply to protected information and security. Services provided by HSA will be welcoming, accessible, integrated, and responsive to the complex needs of people with co-occurring disorders/conditions.
