Critical Time Intervention (CTI) Specialist- Homeless Case Manager

Status of Position: Full-time, exempt/non-exempt, position available in the Residential and Support Services Department at Human Services, Inc. located in West Chester, PA. The CTI Specialist/Homeless Case Manager is responsible for:
Essential Job Functions:
- Provide street/shelter outreach
- Assist consumers in making critical transitions in their lives
- Assist consumers in creating linkages to long term community supports
- Assess and evaluate consumer strengths and needs
- Work with other CTI members in a team approach
- Work in the consumer’s community
- Mediate and advocate on behalf of consumers to help them obtain and maintain permanent housing, health care, benefits, entitlements, and transportation
- Monitor service delivery
- Travel throughout Chester County including Oxford, Nottingham, Phoenixville etc. is required.
Compensation:
Bachelors Degree: $35,000
Master's Degree: $40,000
Required Clearances
FBI, Child Abuse History, PA Criminal History, PA Driver’s License. Employees must maintain a valid driver's license and current automobile insurance.
Minimum Qualifications
A Bachelor’s degree in social work, counseling, psychology, or other social service-related field.
- Have a thorough knowledge of recovery and wellness philosophy.
- Have a working knowledge of issues faced by those who experience serious mental illness and homelessness
- Must possess the ability to communicate effectively both orally and in writing.
- Establish positive working relationships with staff, community partners and consumers.
- Relate positively with colleagues and representatives of other agencies.
- Have the ability to recognize and be sensitive to cultural and ethnic differences.
- 1 year experience with homeless/mental health population
- 1 year TSS/Case Management experience.
