The Manager, Transition Coordination (Care Coaching) leads teams of Transition Coordination staff responsible for the oversight of evaluation of member's needs and requirements to achieve and/or maintain optimal wellness state. The Manager, Transition Coordination works within specific guidelines and procedures; applies advanced technical knowledge to solve moderately complex problems; receives assignments in the form of objectives and determines approach, resources, schedules, and goals.

               

Position Responsibilities:

• Oversees the assessment and evaluation of members' needs and requirements to achieve and/or maintain an optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members.
• Supports and oversees the initial and ongoing member transitions in and out of the various Indiana Medicaid programs, the Contractor’s enrollment, and among care settings
• Makes decisions related to resources, approach, and tactical operations for projects and initiatives involving own departmental area and requires cross departmental collaboration
• Conducts briefings, and area meetings, maintains frequent contact with other managers across the department.
• Supervises a team of 12-15 Transition Coordinators throughout the state of Indiana

Required Qualifications:

• Bachelor's Degree.
• 2 years of managed care experience in Medicare and/or Medicaid.
• 1 years of management/leadership experience.
• Proficiency in analyzing and interpreting trends.
• Comprehensive knowledge of Microsoft Word, Excel, and PowerPoint.
• Progressive experience in the health solutions industry, with emphasis with leading and managing teams.
• Must reside in the state of Indiana.
• Must be able to travel in the state of Indiana to provider offices and Humana locations.

Preferred Qualifications:

• Master's Degree in human services, social sciences, or health related field.
• Applicable State licensure in field of study; RN, LBSW/LSW/LCSW, LMHCA/LMHC, or equivalent.
• Proficiency in analyzing and interpreting financial trends
• Previous experience in utilization management, discharge planning and/or home health or rehabilitation
• Experience with health promotion, coaching and wellness
