QUALIFICATIONS:
Graduate of an accredited school of nursing. Must maintain current Maryland
License as an RN or licensure in a compact state that allows practice in Maryland.
Must maintain current CPR & First Aid certification.
BSN or Bachelor’s degree in Health Care Related Field is preferred; Prior medical case management experience highly desirable. Experience in behavioral health field a plus.
Ability to relate well to individuals served, staff, and board members of different races, ethnicity, genders, sexual orientations, socioeconomic backgrounds, and religions.
POSITION SUMMARY: Responsible for coordinating the somatic care for behavioral health participants assigned to them, ensuring implementation of the treatment plan in partnership with the individual and family, conducting health promotion and education, providing referrals, and offering additional support as needed. Coordinating hospital discharged and community transition. Position requires innovation and “outside of the box” thinking to ensure client outcomes.
Responsibilities:
- Clinical
- Provide injections and administer medication as needed
- Take vital signs as necessary to monitor and report progress on chronic conditions
- Serve as educational consultant on medication and somatic issues to consumers, family and other staff
- Initiate and monitor treatment plans in collaboration with the treatment team.
- Conduct health & wellness groups to help educate clients on health behavior and disease prevention
- Provides leadership and guidance to the treatment team in individualizing and implementing the plan of care
- Provide direct care services to health home participants as necessary
- Case Management
- Take appropriate action to facilitate care to the consumer by assigning, monitoring and following up on tasks delegated to the care team member.
- Coordinate the follow up of treatment plans and hospital discharge plans
- Identify consumer and family needs and arranges for consultation with specialty services needed, including, but not limited to, medical, psychosocial, financial, environmental, procurement of services and discharge planning.
- Serve as an advocate on behalf of consumers and families.
- Collaborates with local hospitals and treatment team to coordinate discharge planning and successful transition into the community.
- Assures follow through on all discharge related activities and functions necessary.
- Acts as liaison to external care providers and community health resources, coordinates patient care conferences, assures completion of all discharge-related functions on a timely basis and assures consumer and family educational needs are met.
- Monitor Health Information Exchange services to identify problems on an individual and population level basis
- Communication and Documentation
- Maintain all required documentation
- Provide other treating professional with relevant information
- Maintain interdepartmental communication.
- Obtain all necessary hospital records to ensure proper communication and follow through
- Follow all agency policies and procedures.
- Agency/Team Cohesion
- Work as a part of an interdisciplinary team to provide quality services to consumers served.
- Responsible for attending mandatory departmental meetings and unit departmental meetings. May be asked to participate in unit-based committees and improvement teams.
- Develop and implement departmental trainings on medical conditions as needed and determined population based data

RESPONSIBILITIES:
- Coordinating the somatic and psychiatric care for clients as assigned
- Creating - with the input of clients, PRP staff, physicians, and nurse practitioner - relevant health and wellness goals and interventions for the Individual Rehabilitation Plan
- Ensuring implementation of the treatment plan in partnership with the individual, PRP Staff, and family
- Providing direct care services as needed in an urgent or emergency situation
- Conducting health promotion, coaching, and education activities via group and individually to clients and staff
- Coordinating hospital or other facility discharge and community transition, post hospital admission
- Monitoring the overall somatic health of clients
- Providing referrals and credible sources of research
- Offering additional support to psychiatric rehabilitation staff as needed
