Practice Based Care Manager

Work Hours: 8 hour daylight Monday - Friday
Physician office based position ensures continuity and coordination of care for HVHS patients, focusing on collaboration with the practice, other providers, and community services. Care coordinator will identify barriers and holistic patient needs including medical, behavioral, pharmaceutical and social needs. Engages patients to develop a comprehensive care plan that includes prevention, member self-management, education, and clinical standards of care. Provides individualized and group education for patients, their families and other interested caregivers.
Required
Bachelor’s degree in nursing (BSN), current Pennsylvania licensure, and other state licensure is required. Three years’ experience with care management of patient physical, behavioral health and social needs, or an equivalent combination of education and experience. Ability to interact with physicians and other health care professionals in a professional manner required. Computer proficiency in Microsoft office products, including proficiency in Excel spreadsheets and analytics. Must have an understanding of health care disparity issues and have the ability to interact with members from diverse backgrounds in a culturally appropriate manner. Excellent verbal and written communication and interpersonal skills required. Ability to use independent judgment and compassion when carrying out tasks.
Preferred
One to two years’ of experience in a managed care environment. Previous practice-base care management strongly preferred.
