What are the responsibilities and job description for the Community Liaison position at Hygeia Home Health?
Hygeia Home Health is currently seeking a Community Liaison to assist our office in Tampa FL area.
The Community Liaison educates our community about our services and programs, builds relationships with accounts for home health referrals and generates revenue, and maintains a positive presence in the community.
Qualifications:
- Home Health/Hospice experience preferred
- Excellent customer service skills
- Excellent verbal and written communication skills
Responsibilities:
· Maintain a current log of all referrals, admissions, non-admits hospitalized patients, and patient insurance. A copy of log will be submitted to the office for payroll and administrative purposes.
· Identify 3-5 new sources to educate on Hygeia's services and specialty programs each month.
· Make appropriate number of calls on prospects verses customers.
· Identify potential growth areas and open new accounts.
· Weekly submission of required paperwork to include items such as expense reports with receipts, in-service sign-in forms, and any other required documentation requested. No exceptions.
