What are the responsibilities and job description for the Visiting Hospice Nurse (RN/LVN) position at I CARE HOSPICE, INC.?
DUTIES
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Provide education and support to patients and their families
- Collaborate with interdisciplinary healthcare team members to ensure comprehensive patient care
- Maintain strict adherence to HIPAA regulations to ensure patient privacy and confidentiality
- Document patient information accurately in electronic health record systems such as HospiceMD
-Participate in coordination of hospice services and reports the identified needs to the interdisciplinary group
-Provide appropriate pain/symptom management and evaluate patient's response to treatments/medications.
QUALIFICATIONS
- Valid nursing license in the state of California
- Minimum of one year of experience as a nurse, preferably in a home healthcare or similar setting
- Strong knowledge of physiology, medical terminology, and disease processes
