Employment Type: Full time Shift: Day Shift Description: The Stroke Program Coordinator is responsible for the organization of services and systems necessary for a multi-disciplinary approach throughout the continuum of stroke care. The Stroke Program Coordinator oversees and manages the stroke program. The Stroke Program Coordinator works closely with patients and families, health care providers, EMS personnel, physicians, co-workers, volunteers and community agencies/service, community employers/employees, and customers. The stroke program services patients over the age of 18 years. Under general supervision of the Clinical Director, (in collaboration with the Director of Clinical and Professional Development and the Area Coordinating Team) and consistent with the Scope and Standards of Practice for Nursing Professional Development (2010), the Clinical Educator is a member of the Mercy Medical Center education team and is accountable for advancing and maintaining nurse competence, enhancing professional nursing practice, and supporting achievement of career goals, building on the educational and experiential bases of nurses across the novice to expert continuum for the ultimate objective of ensuring delivery of efficient, effective, safe, quality, and compassionate patient care. He/she cultivates an environment that promotes an appetite for lifelong learning by supporting the development of effective, quality, innovative, creative, and fun continuing education activities, implementing evidence-based practice innovations, and continuously evaluating further learning needs and/or practice changes. He/she supports a shared leadership philosophy and advances the professional practice model for patient care services. The Clinical Educator must hold an active license to practice nursing in the state of Iowa and maintain continuing education units per state licensure requirements. A BSN or BS/BA and a minimum of two years of clinical experience are required. Experience as a nursing preceptor or in nursing/patient education is preferred. Certification by a nationally recognized nursing organization in staff development and/or an appropriate specialty area is recommended, as is membership in an appropriate professional nursing organization. Basic Life Support Certification (BLS) will be required within 30 days of hire and every 2 years thereafter. Child and Dependent Adult Abuse Mandatory Reporter Training is required within 6 months of hire. Additional Job Description FT M-F position - days. Manages sepsis audits and patient care, attends sepsis meetings and reports off to appropriate teams. Educates and follows up with staff to promote optimal patient care. Develops multidisciplinary stroke educational programs and works closely with the Stroke Program Medical Director in the development and maintenance of a quality stroke program. Maintains effective communication with members of the health care team to achieve desired patient outcomes and evaluating nursing practice. Maintains Joint Commission designation for MercyOne Medical Center as a Primary Stroke Center and ensures that the care of stroke patients meets or exceeds the standards that are required with such designation. Ensures quality assurance and improvement of the stroke program utilizing the Get With The Guidelines (GWTG)- American Heart Association (AHA) stroke database and concurrent patient review to develop a quality improvement plan. Addresses quality issues with nursing, physicians, and EMS involved in the continuum of stroke care. Acts in the capacity as a member of the Core Stroke Team. Preforms audits on code stroke patients and patients admitted with strokes/stroke rule out. Assists in Emergency Department (ED) when needed to help with patient flow. Works with Emergency Medical Services (EMS) Coordinator and EMS Medical Director to improve pre-hospital care for patients. Provides stroke data at monthly staff meetings and ED physician meetings. Leads Stroke Committee every other month where data, areas for improvement, and new ideas are presented to the interdisciplinary team. Demonstrates activity to impact change within the department and with other departments. Promotes and establishes effective relationships within the department, with other departments, physicians, and with hospital guest and clients. Takes an active role in enhancing abilities through personal and professional/job-related growth and development, participates in educational programs, hospital and departmental activities. Performs other duties consistent with purpose of job as directed. Assists with orientation pathway and planning of new hires orientation to the department.
