**Job Summary:** We are seeking a dedicated Case Manager to join our team. The ideal candidate will have experience with the drug and alcohol recovery community. will be responsible for providing support and advocacy to individuals and families in need. The Case Manager will work closely with clients to assess their needs, develop care plans, and connect them with appropriate resources. **Duties:** - Conduct comprehensive assessments to identify clients' needs and goals - Develop individualized care plans in collaboration with clients - Provide crisis intervention and support during difficult situations - Coordinate services and referrals to meet clients' needs - Monitor clients' progress and adjust care plans as necessary - Advocate for clients within the healthcare and social service systems - Maintain accurate and up-to-date case notes and documentation **Qualifications:** - Bachelor's degree in Social Work, Psychology, Counseling, or related field - Previous experience in case management or related field preferred - Knowledge of addiction counseling, crisis intervention, Medicare, child welfare, behavioral health, childcare, mentoring, Section 8, and crisis management - Strong communication and interpersonal skills - Ability to work effectively with diverse populations - Excellent organizational skills and attention to detail This is a rewarding opportunity for a compassionate individual looking to make a positive impact on the lives of others.
