What are the responsibilities and job description for the Care Manager position at IGHL/ Angelas House?
The Care Manager is responsible to assist families and children with identifying, assessing, planning and monitoring a comprehensive Plan of Care.
Various days and hours.
RESPONSIBILITIES:
E- Connotes Essential Functions
E-1 Develops and implements the plan of care for families with input from the physician, family and all other pertinent personnel every 180 days
E-2 Maintains up-to-date Physician’s Plan of Treatment (doctor’s orders) every two months
E-3 Responsible for monthly visits to meet with the child, siblings and parents to monitor and report progress, problems, setbacks, crisis, etc.
E-4 Coordinates all services and service providers. Obtains progress reports and intervenes with service providers in place of parents or in conjunction with parents and their desires and goals for their children
E-5 Interacts with insurance companies, vendors and contractors to obtain services and projects for the child’s health and well being. Acting with parent’s consent and knowledge of their wishes
E-6 Provides comprehensive care management to the child and whenever necessary advice and support the entire family as a unit
E-7 Maintains legal documentation of all communications with all service providers, family members or anyone with whom the case manager interacts with on the part of the child and the family
E-8 Maintains at least weekly contact with a family member to monitor problems, progress, etc.
9- Complete a CANS assessment every 180 day
10- Participates in the development of the child’s educational plan, which may include subsequent transfer of payment responsibilities for certain therapies and equipment
11- Reports changes to service providers as needs and available resources fluctuate. Adjusts the corresponding plan of care as well as negotiate coverage with potential payers
12- Maintain the annual Level of Care Eligibility
13- Maintain the Health Home responsibilities such as a mandatory visit, mandatory contacts, documentation and any other duties assigned
14- Timely submission of all documentation and paperwork
15- Attends mandatory Health Home and staff meetings
16- All other duties as assigned
REQUIREMENTS
A. Bachelor’s degree in related field preferred
B. Nursing license or related field experience
C. Excellent communication and interpersonal skills
D. Knowledge of individual and institutional services available in the community, i.e. medical, social, educational, etc.
E. Possess a thorough knowledge of entitlement programs and funding sources, both public and private, as well as applicable eligibility criteria
F. Demonstrate advocacy and brokering ability
G. Have a proactive approach to problem identification and resolution
H. Valid NYS Driver’s License
Job Type: Full-time
Pay: $48,000.00 - $52,000.00 per year
