Principal Duties And Responsibilities
- Builds and maintains professional relationships with all levels of care providers and healthcare professionals, as well as discharge planners and other relevant hospital or physician staff. Helps to personally shepherd a positive customer experience and successful outcomes.
- Ensures that relevant hospital and physician relationships know and understand services and products provided by SKLD.
- Has a working knowledge of the hospital and its various departments regarding what departments generate patient awareness about the medical specialties and services provided at SKLD and the sub-acute level of care.
- Identify and pursue opportunities for strategic business development.
- Attends and/or coordinates events at the SKLD facilities.
- Oversee all referral management-related activities at your accounts in conjunction with the Admissions Department.
- Assesses patient needs through chart review and where necessary direct patient contact.
- Secures required level of care determination information and completes and distributes intake paperwork.
- Coordinates availability issues with SKLD sources and seeks necessary approvals to reach a timely patient decision.
- Meets with patients and families to explain and educate regarding SKLD services and products.
- Provides management with facility information that may impact future patient education or healthcare awareness strategy.
- Organize and present SKLD at sponsored events.
- Assist in creation and distribution of media strategies and campaigns.
- Report on activities and market dynamics.
- Build stable census in the facilities by working toward aggressive patient volume goals.
- Maintains up to date knowledge of SKLD services and capabilities.
- Facilitates concerns between Liaisons and facilities.
- Regular and reliable attendance.
- Performs other duties as assigned or requested, from time to time, by the supervisor or management.
Required/Desired Qualifications:
Education, Training, And Experience
- Knowledge and successful experience in the healthcare setting, 2 years preferred.
- Bachelor’s degree in Nursing, Healthcare, Business Administration or other healthcare related field preferred.
- Must be self-motivated and goal-oriented.
- Knowledge of nursing standards and delivery systems, management, staff development and cost containment.
- Ability to communicate effectively, verbally and in writing with all levels of health care professionals and support staff.
- Ability to work independently.
- Strong critical thinking and problem-solving skills.
- Display positive professional image and enhance positive public relations for the Corporation.
- Maintain confidentiality of patient information.
- Basic business computer & Smart Phone knowledge preferred.
