Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13 facilities with 22 micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.
Job Description
Case Manager is responsible for assessing the strengths and needs of clients, linking clients with
appropriate resources, creating housing and service plans, monitoring clients’ progress towards goals
established in service plans and empowering clients towards self-sufficiency. Case Manager is
responsible for practicing active listening, providing trauma-informed care, and acknowledging and
documenting client progress.
The pay range for this position is $22.00 - $25.00 per hour, depending on experience.
Responsibilities
- Develop a service plan, evaluate the households’ needs, and establish short-term goals
- Reassess households’ needs and goals regularly
- Develop and maintain a network of resources with other community-based providers
- Find appropriate resources for households; research and understand best practices
- Support households with referrals to appropriate resources and provide follow-up to ensure that linkages are successful
- Ensure households have appropriate medical insurance and are linked to a medical provider
- Work cooperatively with Housing Navigator and other departments at illumination Foundation to ensure that clients successfully transition into permanent housing. Assist with housing move-ins.
- Ensure clients participate and are engaged in IF activities
- Consult with parents regarding parenting, child development, school attendance and other familial concerns. Provide resources for children as needed.
- Monitor children for signs of emotional or developmental troubles and communicate to supervisor
- Monitor for signs of emotional, physical, and sexual abuse and communicate to CPS as deemed necessary by law.
- When attending appointments with families advocate for clients relating to housing, health, education, and social and emotional wellbeing.
- Provide transportation for client appointments if deemed necessary.
- Conduct regular home visits and complete wellness checks
- Ensure residency maintains habitability standards
- Attend individual supervision sessions to discuss the program, organization, client’s progress, new strategies to meet client’s level of need and improve service plan
- Attend and actively participate in case conferences and other team meetings
- Maintain updated & accurate case notes, records, & relevant paperwork on KIPU & HMIS.
- Establish and maintain confidential case files for participants and prepare required statistical reports for program management and evaluation.
Preferred Experience/Minimum Qualifications
- Bachelor’s degree from an accredited college or university in Social Services / Healthcare or related field or equivalent combination of training and experience.
- Experience in the area of non-profit housing and/or housing for people with disabilities and chronic health conditions.
- At least 1 years of experience working with in homeless services, case management, mental health, permanent supportive housing experience
- Previous experience working as Case Manager or similar position
- Maintain and execute confidential information according to HIPAA standards
- Current valid CA driver's license
- Have a dependable vehicle with automobile insurance
- Computer literate; ability to use Microsoft Office products and databases such as Help
- Management Information Systems (HMIS).
- Ability to use Google products.
- Strong written and communication skills
- Bilingual in Spanish and English is helpful, highly desirable
