Care Coordinator for Chronic Care Management

We are committed to enhancing patient engagement and retention by providing patients and clinics with technology monitoring, coaching, and personalized care plans. We are expanding our services to the East Coast and hiring a Care Coordinator and Medical Assistant.

Job Description:

· Care Coordinator - RN Licensing Required

- Facilitate CCM care coordination to ensure all parties work towards common goals.

- Develop personalized care plans with medical groups and their healthcare providers.

- Collaborate with patients and clinics to access, plan, implement, monitor, and evaluate care options.

- Review and update care plans based on clients' evolving needs and conditions.

- Evaluate the team and team members' performance required to meet an individual's health needs.

· Medical Assistant: LVN Preferred

- Monitor the patient on the platform and maintain accurate and updated records of patients' assessments.

- Serve as a primary point of contact for patients, families, and clinics.

- Provide patients and families with education and ensure that patients understand the daily basic care requirements.

Job Types: Full-time, Part-time

Pay: $30,000.00 - $40,000.00 per year

Expected hours: 24 – 40 per week
