Responsibilities:

 

·         Supervise all Hospice and Home Health Social Workers.

·         Implementation of standards of care for social work services

·         Conducts interdisciplinary patient assessments and addresses psychosocial needs.

·         Participates in the coordination of services with other clinic team members and community resources as needed

·         Manages a patient caseload as needed providing counseling, crisis intervention, and social evaluations.

 

Requirements:

·         Clinical Social Worker State License

·         Master’s degree in Social Work (MSW)

·         Clinical experience practicing as a LCSW

    • Management experience

·         Hospice and/or Home Health experience preferred

·         Compassion and integrity
