Seeking Full time RN to manage Chronic Illness - Transition of Care Management clinic for a group for a large Internal Medicine group. Role will include follow up calls, managing medications, routine exams while managing vitals and care. Part time position with potential to be full time. Must have active license and certification.

Pay: From $35.00 per hour
