Skilled nursing experience with the geriatric population is required for this position.

Job Responsibilities
- Facilitates discharge planning from time of admission.
- Completes inital assessments such as social history, care plan, trauma screening and MDS.
- Leads care conferences with patient, family and interdisciplinary team.
- Issues notices of Medicare non-coverage, arranges skilled homecare services, orders necessary medical equipment and arranges transportation as needed.
- Knowledge of geriatric population.
- Must be able to relate information concerning a patient's care plan/progess.
- Coordinates with interdisciplinary team to address patient/family grievances.
- Basic understanding of following step-by-step procedures.
- Follows and understands written and oral directions.
Education & Qualifications
- Must possess at a minimum a bachelor's degree from accredited school in Social Work or related field.
- Experience in Skilled Nursing is required.
- Must be knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care.
- Ability to work independently or part of a group.
- Strong communication skills.
- Must have patience, a cheerful disposition and enthusiasm, as well as the willingness to handle challenging situations.
- Effective problem solving skills.
- Must possess the ability to deal tactfully with personnel, resident, family members, visitors, government agencies/personnel, and the general public.
