We are seeking an exceptional Pharmacist to lead our Chronic Care Management (CCM), Transition Care Management (TCM), and Remote Patient Monitoring (RPM) services. This full clinical service program will operate through a Collaborative Practice Agreement with independent medical facilities in the West End, NC area.
As a Clinical Pharmacist, you will establish and maintain professional relationships with all team members within the medical practice. Responsibilities may vary depending on the unique needs of each independent practice, offering you a diverse and stimulating workload. Must live in a commutable distance to the posted job location – this role is performed in provider clinics and may require regional travel to a group of clinics.

In This Role, You Will Have The Opportunity To
- Build and work directly with your patient panel to drive clinical outcomes and results.
- Spearhead patient enrollment, device management, and data interpretation for RPM.
- Collaborate on medication management with supervising providers.
- Participate and lead topic discussions for current staff and residents.
- Precept PGY-1 Students and participate in In-Clinic Rx Fellowship Program.
- Develop individualize care plans based on specific patient needs.
- Provide recommendations or implement therapy changes, when necessary.
- A current, valid license to practice pharmacy in North Carolina.
- Current CPR certification.
- Certification for Immunization.

- PharmD Degree with three years of experience.
- Currently, hold a Board of Pharmaceutical Specialty Certification or Completion of APhA/ASHP Accredited PGY-1 or PGY-2 Residency Program.
- Completion of an APhA Certificate Training Program.
- Monday through Friday 8:00 am – 5:00 pm (No Nights or Weekends)
