We have a unique opportunity for a Virtual Nurse to join our team as we implement a new patient care model that leverages technology to provide additional support to our staff while also improving patient experience. As a Virtual Nurse, you will utilize your clinical knowledge, critical thinking skills, and the principles of case management and adult education to coordinate, implement and support a plan of care that meets the patient's needs and ensures a seamless, effective, and efficient transition of care. This includes the assessment, planning, implementation, coordination, monitoring, and evaluation of patient plan of care from admission to discharge. This is an onsite position – you will work from a location within the hospital campus. GENERAL REQUIREMENTS • Must be licensed in the State of Indiana as a Registered Nurse. • Bachelor of Science in Nursing preferred. • Graduate of an accredited school of nursing. • Certification by a nationally recognized nursing organization is preferred • Basic Life Support for Healthcare providers certification • Minimum ten (10) years of experience in health condition management, which must include adult condition management.
