Our case managers are responsible for the coordination and ongoing facilitation of participant maintenance of home and access to community environments, as well as the management and training of staff. Responsibilities
- Perform participant assessments upon intake and on an ongoing basis
- Facilitate the development of participants' Circles of Support
- Responsible for case management of participants' file
- Reviews all collateral material obtained at admission and on an ongoing basis
- Coordinate the development of participants' Individual Support Plans (ISP)
- Provides training, support, and direct assistance if not available through more natural means
- Secures access to environments by coordinating, developing, securing, or providing necessary supports/technology
- Coordinate the delegating of assistance to the participant whether through paid or natural supports
- Monitor the health and safety of participants
- Facilitate the quality assurance of participants' ISP
- Initiate, implement emergency assistance, responsive system for participants
- Accept on-call responsibilities as assigned that include weekends and evenings
- Monitor all services provided to participants, and the competency and overall effectiveness of contracted staff and direct personnel
- Provide assistance as requested with acquiring roommates, attendant care for participants, training, and support
- Complete satisfaction review with participants no less than quarterly.
- Complete required paperwork (i.e. staff time cards, Annual Review, monthly case notes, and performance evaluations)
- Provides direct personal care to participants as needed (i.e. feeding, toileting, etc)
- Facilitate participant and team between Regional Center, families, landlords, etc.
- Attend staff and in-service training as directed
- Other duties as assigned
Qualifications
Qualifications
- Bachelor's degree in a human services or equivalent experience is preferred
- Must have one year of direct care experience
- Two years of management experience preferred
- Must possess a valid CPR and First Aid certificate in order to physically provide for participants' safety and well-being (i.e. lifting, transferring, positioning, etc.)
- Must be willing to be on-call evenings and weekends on a rotational basis
- Employee must submit to a criminal record clearance
- Employee must have a valid CA Driver License and a clean driving record (within InAlliance's guidelines) from the Department of Motor Vehicles
