FAMILY PRESERVATION FACILITATOR

General Responsibilities

The Family Preservation Facilitator is responsible for assessing the appropriateness of the referral to the program. They will be engaging a youth and family in the family preservation services as outlined in the DCS Service Standard. They will be developing a crisis plan, providing weekly safety checks, and stabilizing crises. They will do this by exploring strengths, needs, culture, and vision with the family. Through collaboration with the family and DCS they will establish Specific Plans of Action including Family Level Objectives and Individual Level Objectives. They will be responsible for monitoring progress toward those goals and objectives established, and ensuring the team effectiveness by collaborating with team members to stay on track with the assigned action steps and by inspiring them to work as a cohesive group toward the family vision. In addition, they will create a transition plan including post-transition crisis management. They will be responsible for ensuring the team has an opportunity to celebrate successes.

Qualifications

Qualifications include having a Bachelor Degree in social work, counseling or related field; or an Associate’s degree with a minimum of 1 year of experience, excellent organizational and communication skills; and a willingness to be trained in and committed to Solution Based Casework principles.

1. Direct Clinical Service Activities
  • Consultation and collaboration with community systems to facilitate linkage referral, and advocacy ensuring families have access to the services and resources needed to reach goals and objectives.
  • Teach youth and families the skills needed to facilitate their family team toward reaching their vision.
  • Assess strengths and underlying needs of families.
  • Identify family skills and develop family skills to prevent child maltreatment.
  • Provide weekly family safety checks as outlined in the
  • Partner with family, youth, DCS, and other providers and natural supports to identify underlying needs and mitigating strengths to facilitate the development of an Specific Action Plan with Individual and Family Level Objectives.
  • Monitor progress toward meeting established goals, and adherence to action plans.
  • Assists in the development of natural supports and non-traditional community resources needed for youth and their families to reach their vision.
  • Assist families in accessing needed community resources and services. Working with the Child and Family Team on making sure Concrete Assistance needs are part of planning.
2. Community Services Activities
  • Maintains networking and referral contacts with all community partners. Provides on-going communication with all involved providers, especially the assigned DCS Caseworker.
  • Serves as contact person for requests for services from referral sources both internal and external.
  • Provides educational programs in the community, as needed.
  • Attends DCS Child and Family Team Meetings.
3. Staff Development Activities
  • Actively participates in the Center’s Staff Development Program by fulfilling identified items on the Performance Evaluation.
  • Participates in team meetings, full staff meetings, etc, as assigned.
  • Completes Solution Based Casework Training.
  • Completes all trainings as assigned in Relias Learning.
  • Attends conferences, workshops, meetings, in-services, etc. to enhance professional knowledge and development according to the need identified on the performance evaluation.
  • Completes CANS Training.
  • Participates in a Trauma Informed Care Training.
  • Serves on committees as assigned.
  • Demonstrates active involvement in supervision as evidenced by constructive participation, initiative and effort with both administrative and clinical supervisor.
  • Schedules, documents, and implements training and in-service activities for staff assigned to IFS program as appropriate.
4. Record Keeping Activities
  • Completes various administrative forms to assure fair and accurate data collection for billing, fee setting, MIS, and other purposes in accordance with agency and program policies and procedures.
  • Completes attendance reports, PTO requests, travel vouchers, staff development requests, self-performance evaluations, etc., accurately and efficiently in accordance with agency policies and procedures.
  • Meets all documentation requirements required by the DCS Service Standard of Family Preservation.
6. Other Duties
  • Performs other duties as assigned.

Knowledge and Skills Required
  • Demonstrates ability to implement good communication skills with a broad range of people.
  • Understanding and philosophical agreement with Solution Based Casework tenets.
  • Must have a valid driver’s license with a good driving record and meet other agency standards regarding operation of Center’s vehicles.
  • Knowledge of community resources in a rural area.
  • Knowledge of and experience with current best practices for families.
  • Demonstrates sensitivity to cultural issues in a rural, poverty area.
Special Requirements for Position
  • Extensive experience working with children with serious emotional disturbances.
  • In-depth knowledge of systems (i.e. schools, child welfare, juvenile justice) involving children with SED.
  • Able to develop and maintain effective relationships with all stakeholders including the child, families, and other service providers.
  • Flexible work schedule to be set with supervisor, but requires a willingness to work evening hours as needed to meet with families (flexibility is especially important regarding weekly safety checks)
  • Advocacy and negotiating skills
  • Willingness to travel on behalf of agency business
  • Willingness to provide services in the schools, homes, and communities of persons served
