Position Description
Valid Registered Nurse (RN) license/certification in the state chosen)

Are you comfortable traveling to patient's homes? (yes)
The work schedule is M-F from 8 AM - 5 PM. Are you available and comfortable with this schedule? (yes)
You will be travelling within a one-hour travel radius from the main location within which you will be visiting patients in-home. 80% of the job is on the road visiting patients. Are you comfortable with this and willing to put in the road time? (yes)
preferred
2 years experience in any of the following:
Dialysis Care
Home Health Care
Hospice
Case Management (CM)
Are you bilingual English/Spanish? (yes)

Required skills & experience: 1) Graduate of an accredited School of Nursing. 2) Currently licensed as a Registered Nurse in the State of MD other NLC state). 3) 2 years previous experience working in care management and/or with CKD/ESRD patients is preferred. 4) Ability to take call remotely on some nights and weekends 5) Must have Home Health and/or Hospice experience.
