Overview

Care Alliance Services, as a care transition program, is a cornerstone to Accountable Care. This position exists to facilitate post acute referrals to statewide home care agencies or extended care facilities. The position will be a liaison between Throughput Services and the post-acute facilities, ensuring that patient/family needs are communicated in a timely and efficient manner. This position will work closely with outside agencies in and out of the Care Alliance Services network of quality providers to ensure services are established with minimal delay or impact to patient care.



  • Requires high school diploma.
  • Requires significant relevant work experience in business or related field.
  • Requires strong interpersonal skills to promote pre-eminent service quality and promote Care Alliance Services positively within the healthcare delivery system and the external community.
  • Requires ability to effectively interact with a widely diverse population of customers.
  • Requires knowledge of payor practices and/or guidelines.
  • Requires experience in a clinical setting and familiarity with medical vocabulary.
  • Requires experience with databases or spreadsheets.
  • Requires ability to communicate clearly and concisely through written and verbal communications.
  • Requires the ability to prioritize and meet deadlines with great attention to detail and organization.
  • Requires ability work will under critical and/or time-sensitive situations.
  • Requires ability to work with all customers; including physicians; throughput services, and external agencies in an efficient and professional manner.
  • Requires ability to assess problems and make decisions.
  • Requires ability to work independently and as a team member.
  • Requires ability to recognize opportunities for improvement and support change.

Salary : $32,600 - $41,200
