Summary:
As a Case Manager in Putnam, CT, you will be responsible for coordinating and facilitating the care and support services for individuals or families in need. You will work closely with clients, healthcare providers, and social service agencies to ensure that the appropriate resources and assistance are delivered to promote the well-being and recovery of the clients.
Responsibilities:
Client Assessment: Conduct comprehensive assessments of clients' physical, emotional, social, and financial needs to develop a tailored care plan.
Resource Coordination: Identify and connect clients to available community resources, social services, healthcare, and financial assistance programs.
Advocacy: Act as an advocate for clients, ensuring their rights and preferences are respected throughout the care process.
Care Plan Development: Collaborate with a multidisciplinary team to create and update care plans that address clients' short-term and long-term goals.
Monitoring and Follow-up: Continuously assess the progress of clients, make necessary adjustments to care plans, and provide ongoing support and education.
Documentation: Maintain accurate and up-to-date client records, ensuring compliance with legal and regulatory requirements.
Crisis Intervention: Respond to emergency situations and provide crisis intervention as needed.
Communication: Maintain clear and effective communication with clients, their families, and other healthcare providers.

Evaluation: Evaluate the effectiveness of the care plans and adjust them as needed to achieve positive outcomes.
Qualifications:
- Bachelor's degree in a related field (such as social work, nursing, or psychology) is typically required. Some positions may require a Master's degree.
