Family Nurse Practitioner

• Position Title: Family Nurse Practitioner
• Location: Chinle, AZ
• Duration: Permanent
• Shift: 8 am to 5 pm
• Schedule: Monday to Friday

Job Duties:

• Work under the administrative supervision of MD or Primary Care Director.
• Provide professional medical services or direct patient care services.
• Aappropriately and timely medical services in accordance with the standards of care established by recognized medical care organizations and in accordance with facility policies and procedures.
• Interview and examine patients, review past medical history and request or perform diagnostic tests and examination deemed necessary to obtain all possible information related to each case.
• Conduct outpatient clinics, make observations appropriate to the outpatients underlying disease and treatment plan, including current symptoms.
• Provide telemedicine services for patients with mild respiratory illness in the CSU Respiratory Clinic.
• Shall provide telephone visits for patients in need of chronic care management for whom it would be high risk to travel to the hospital for care.
• Advise the medical officers in other services concerning these operations.
• Provide appropriate documentation on rendered patient care utilizing facility EHR system.
• Adhere to Safety management reporting requirements in accordance with facility Safety Measure Policy.

Required Qualifications:

• License as a Family Nurse Practitioner (FNP)
• Master's degree in Nursing with specialization in Family Practice or related field
• Current certification as a Family Nurse Practitioner by a recognized certifying body (e.g., AANP or ANCC)
• Minimum of 2 years of experience as a Family Nurse Practitioner preferred
• BLS and ACLS certifications
• Strong communication and interpersonal skills
• Ability to work collaboratively in a team-based healthcare environment
