- ASAP*Benefits-** Health (Medical, Dental, Vision) and 401K Benefits for full-time employees* Competitive Personal Time Off program for leaders* Employee Assistance Program mental, physical, and financial wellness assistance* Professional development and Advanced Degree support* And much more*Job Duties-** Provide leadership and management for case management across hospitals within the Highpoint Health market.* Oversee and delegate day-to-day operations, including directing and planning.* Formulate and implement operational strategies and initiatives in the case management program.* Ensure staff competencies in utilization review, transition/discharge planning, and social work services.* Offer specialized expertise and ensure resource alignment with system-wide initiatives.* Update relevant parties on case management strategies, initiatives, outcomes, and issues.* Collaborate with physicians to ensure delivery of evidence-based care and reduce avoidable clinical variations.* Partner with Hospital Executive Leadership, Nursing, Medical staff, and other department stakeholders to facilitate care in a safe environment.*Requirements:** Bachelors degree in Nursing
- Required* State of Tennessee or Multi-State Registered Nurse licensure
- Required* Minimum four years management experience directing a Case Management Department in an acute care hospital.* Working knowledge of Care Management department functions including Joint Commission and Conditions of Participation (COP) requirements.* Good understanding of Medicare, Medicaid, and other third-party payment systems.* BLS certification from the American Heart Association (AHA) or American Red Cross (ARC)
