Case Manager
Location: Harlem, NY (onsite 1x every other week)
Duration: Direct Hire - Permanent
Salary Range: $55-64k depending on level of experience
Must Haves:
- Bachelor’s degree in relevant behavioral/social sciences field required
- 1-3 years of case management experience
- Experience conducting home studies according to state requirements required
- Strong professional writing, particularly for assessment reports, required
- Knowledge of local community social services required
- Experience working with immigrant populations required
Responsibilities:
- Complete home studies and in-person case contacts
- Ensure safety for all youth on caseload
- Manage a high turn-over caseload
- Comply with agency and regulatory policies and procedures
- Coordinate initial intake for each youth
- Coordinate and handle an estimated caseload of youth at any given time (HS and PRS cases)
- Coordinate, schedule, and confirm necessary services for each youth in their location
- Communicate regularly with the youth and sponsor to address any needs and/or concerns
Salary : $55,000 - $64,000
