MAJOR FUNCTION: This position is responsible for providing coordination of care within the Inspira Hematology & Medial Oncology department, working under the direction of the practice manager. The coordinator will serve as a liaison between the patients and the multidisciplinary care team and will coordinate care across the health network to keep patients moving through treatment. Using strong critical thinking skills, the coordinator will assess and remove barriers such as transportation and recommend appropriate interventions and support services to meet the needs of the patient. The coordinator will meet patients to develop care initiatives and will work in conjunction with the patient’s multidisciplinary care team to provide a high-quality patient experience. Will provide continuity and coordination of patient care and track patient outcomes and relevant data throughout the system in regard to various cancer programs and initiatives. Participates in educational and community activities and assumes responsibility for ongoing growth within the role. Serves as a resource and expert in new technologies offered within the cancer center. Coordinates various cancer care and screening programs as directed. This position will also act as a point of contact regarding appointment scheduling, pre-authorizations, and registration of related appointments and procedures for patients. Also assists patients with Presumptive Eligibility, Family Care, Medicaid and Charity Care Programs as appropriate.
QUALIFICATIONS:
A. Education: High School Diploma or equivalent required, Associates Degree in healthcare preferred.
B. Experience:Minimum of 3 years of experience working with patients in an Oncology practice or setting required. One year medical/surgical scheduling experience required. Knowledge of presumptive eligibility; family care, Medicaid and/or charity care programs preferred. Experience working with the public, computer data entry, Strong verbal and written communication skills. Knowledge of Cerner EMR preferred. Knowledge of Intellidose Chemotherapy management tool preferred. Must have the ability to communicate effectively with patients, families and other departments to build trusting relationships; must be able to demonstrate empathy, integrity, honesty and compassion; and have the ability to build trust with patients, families and care team by being accessible, accurate and supportive while working within the scope of practice
