What are the responsibilities and job description for the Program RN – Home Care position at Inspira Health?
The region’s leading network of healthcare providers, Inspira Health, is seeking a Program RN – Home Care to provide home care services to participants of Inspira LIFE in Williamstown, NJ. Inspira LIFE offers seniors, with ongoing health needs, individualized health care plans tailored to meet their needs that may include a balance of both home-based and center-based care.
Responsibilities:
Under the supervision of the Clinic RN Manager, provide home care services to Inspira LIFE participants
Perform initial and annual assessments of participants in their home care environment and coordinate plan of care
Review plan of care and evaluate participant compliance
Provide skilled assessments, treatments and health education to participants in their homes
Maintain the participants’ medical records with thorough documentation and communicate participants’ changes and progress to appropriate staff
Qualifications:
BSN from a professional school of nursing
Minimum of 1 year working with the frail or elderly and 2 years of nursing experience working in LTC, acute care or community nursing
Current New Jersey RN License
CPR certification
Valid driver’s license, proof of insurance and reliable transportation
