Summary/Objective
Care Partners provide routine care in a manner conducive to the comfort and safety of residents in accordance with federal, state, and local regulations and within the established policies and procedures of the Community. S(he) is delegated the responsibility in carrying out the assigned duties by the Health & Wellness Director, Executive Director, Charge Nurse or other assigned supervisory personnel to assure that the highest degree of quality compassionate resident care is maintained at all times. Care Partners are customer service experts who interact with leadership, families, vendors, guests, and other associates while displaying exceptional professionalism.
Essential Functions
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, stooping and crouching all day. The employee must frequently lift or move objects and patients weighing over 50 pounds. The noise level is moderate to quiet. Because the essential functions may require exposure to bodily functions and infectious waste, diseases, odors and other situations associated with health care, including but not limited to exposure to AIDS, HIV and Hepatitis B viruses involving Risk Classification Categories 2 and 3**, environmental and safety conditions will fluctuate. To be qualified for the position of Care Partner, an applicant/employee will need to possess the ability to work in an environment conducive to caring for residents without posing a substantial safety or health threat to ones self or others.

Care Partners may hold Med Tech certifications (optional) or Certified Nursing Assistant (CNA) (optional) but must have a High School Diploma or equivalent. It is preferred that the Care Partner has six months experience working in a memory care facility, nursing home, hospital, home health agency or other health care facility. S(he) should also possess exceptional communication and customer service skills, both written and verbal as well as strong planning and organizational skills. S(he) should also exhibit exceptional teamwork skills, bedside manner, infection control, and health promotion and maintenance. The utilization of Electronic Medication Record keeping is required.
