Assessment Coordinator
The Assessment Coordinator maintains responsibility for providing support to the Agency-Directed Services Nursing Department and completing activities related to Missouri Medicaid In-Home and Consumer Directed Services authorized participant assessments.
Must become a qualified assessor completing the MO DSDS online reassessment training and passed the exam to receive a Training certification;
Monitor and manage list of reassessments;
Assesses living arrangements, physical capabilities, cognitive abilities, medical requirements, and financial resources to determine level of care required and eligibility for Medicaid-funded services using the CyberAccess Web Tool within completion of the assessment on or within 5 business days of completion of the reassessment, and no later than the last day of the month preceding the month the participants care plan is scheduled to expire, with oversight from Agency-Directed Nursing as needed;
Ensure each reassessment is complete and through per MO DSDS guidelines
Travels, as needed, to conduct home visits and attend hearings, meetings, and training sessions;
Notifying the DSDS Provider Reassesor Review Team immediately once a reassessment . and all required documentation is completed by encrypted email;
If determined by the Provider Reassessor Review Team the reassessment is incomplete, the review specialist will make contact via encrypted email for additional information and/or remediation;
Completes request by Review Team of any additional information and/or remediation in three business days;
Follows established departmental policies, procedures, and objectives ;
Maintains required reports, being responsive and proactive to concerning trends;
Observes, documents and timely reports concerns in client condition to the appropriate personnel and completes mandatory reporting, “hot-lining”, when appropriate, as required by law;
