I also get a lot of support from the team — they're the best" "I love IntelyCare because there are always shifts available and the rate is always fair for everybody" **Job Responsibilities** • Deliver post-acute care as an LPN • Monitor the physical and mental wellbeing of residents and track their vital signs • Perform intakes of residents and document their health concerns and medical histories • Review medical records and charts of residents • Properly administer and verify medications • Maintain appropriate levels of medical supplies • Contribute to a safe, comfortable environment for residents • Assist with IV set up, vaccinations, and phlebotomies • Be accountable to follow and adhere to care plans for residents • Help medical staff with orientation and training **Qualifications and Skills** • Current Ohio state license to practice as a Licensed Practical Nurse (LPN) • Completion of approved Licensed Practical Nurse (LPN) educational program • H.S. diploma or GED (or equivalent) • Ability to lift a minimum of 50 lbs • Minimum of 6 months of experience (LPN or related role)Salary : $30 - $58
