Summary;
Our medication management prescribers will be working in a true integrated model; supported by a medical assistant and a care coordinator, our prescribers will be able to devote their encounters to managing their patient’s medication. Psychiatric APRNs and MDs will have the ability to provide warm hand offs when patients are in needs of additional behavioral health or physical health care.
Essential Duties & Responsibilities;
- Performs specialized duties in providing medical and psychiatric care to children and/or adults depending on certification and privileging
- Provides appropriate nursing and/or health care services to clients including clinical assessments, treatments, diagnosing medical conditions, assessing emergencies, ordering diagnostic studies, prescribing medications, leading cardiac arrest codes and documenting medical care
- Performs and documents appropriate psychiatric medication evaluation and management services to patients including psychiatric medication evaluations and documents with appropriate E&M codesCompletes all related charting in the agency’s Electronic Medical Record within 24 hours
- Maintains patient productivity standards of 14-18 patients dailyProvides referrals for other medical services including referrals to InterCommunity’s primary care department, outpatient therapy services, community support/rehabilitation services, detox, or residential programs when appropriate
- Provides direction to staff on appropriate health record documentation
- Consults with clinical staff regarding assessment methods and treatment plans
- Consults with other departments including onsite pharmacy to ensure appropriate care is being offered to patientsCompletes appropriate paperwork or refers patients to appropriate alternate who can assist with paperworkParticipates in quality assurance and other clinical committees as needed
- Works well within a team environment and helps promote positivity among the teamEngages in team meetings and other training programsFunctions as liaison between agency and community health care providers
- May assist families and other groups in understanding medical needs of clients serviced by agency
- May provide training to nursing and paraprofessional staff including practicum studentsrespond to common inquires or complaints from customers
- *All agency staff are required to attend all mandatory department/agency meetings and trainings*
Requirements
Education &/Or Experience;
Master’s degree or equivalent; or 4 to 10 years related experience and/or training; or equivalent combination of education and experience. A current license as an advanced practice registered nurse issued by the Connecticut Department of Public Health. Considerable knowledge of nursing principles and clinical therapeutic models; considerable knowledge of relevant state and federal laws, statutes and regulations; knowledge of state, federal and local health care agencies and programs; knowledge of pharmacology as it relates to advanced nursing practice.
