A few must-haves for Hospice Medical Social Workers:
- Master’s degree in social work and active MSW license in SC.
- Minimum of 1 year of social work experience, ideally in hospice.
- Knowledge of state and federal hospice regulations.
- Strong interpersonal skills, good communicator, empathetic, compassionate, and resourceful.

As a Hospice Medical Social Worker, here’s a big-picture view of what you’ll do:
- Psychosocial assessment in a home-based setting. Assessing the needs of patients and families. Documenting the plan of care, visits, and updates.
- Work with a hospice IDG team of dedicated workers – RN, CNA, Chaplain, and Medical Director providing psychosocial status updates and needs.
- Provide counseling and support in grief, community resources, intervention for mental and emotional barriers to achieving goals of care. Special needs related to religious or cultural diversity. Advanced care planning goals for patients and families.
