Schedule: Monday-Friday 8:00am - 5:00pm

Hourly rate: $17 per hour, plus on call compensation, mileage reimbursement for travel, and bonus potential

Location: 2000 Vermont Dr Suite 100, Fort Collins, CO 80525

We are actively looking for a dynamic person to fill our Personal Care Coordinator position in our Personal Support Services Department (PCSS). This role provides client care coordination and serves as a single point of contact for clients, families, case managers and providers to ensure expectations are met or exceeded. Responsibilities:

• New client Intake
• Initial and subsequent screening assessments, in clients homes, every 90 days
• Identify resources to address client’s health, daily living, and safety needs’.
• Perform administrative tasks associated with service to clients including scheduling, logging, documenting all appointments and services.
• Provide information, notices and regular communications with clients and designee.
• Monitor client well-being, recommending resources and solutions as appropriate. Set up all client appointments

Requirements:

• Minimum of 18 years of age.
• High school graduate or equivalent
• Previous experience in the home care industry or closely related personal care services 
• Previous experience supervising employees preferred
